Activity Referral Scheme

Our activity referral scheme is a programme of physical activity designed to help improve your health and quality of life. 

Your local health professional can refer you to this programme.

Who can access the Activity Referral Scheme?

The scheme is open to people who want to get active, over the age of 16 with a stable health condition. Our team are highly trained therefore regardless of the type of condition you may have we will always offer a programme of physical activity suited to your health needs.

Our instructors will support you throughout your time with us and will contact you to discuss your programme and make sure you are getting the most benefits from it. We also offer an open door policy therefore you can pop in or call us anytime for a chat about your programme.

We offer a wide range of activities including specialised classes, seated exercise, with some activities outdoor or even in the water. They are available all across the Selby District and take place in leisure centres and community venues, such as church halls. We regularly see participants with:

  • Heart problems
  • Back pain
  • Diabetes
  • Cancer
  • Lung conditions (Asthma, COPD)
  • Stroke, Parkinson’s Disease, MS
  • Arthritis and mobility problems
  • And many others

How do I Join the Activity Referral Scheme?

You can either ask your local Health Professional such as your GP, Nurse or Physiotherapist for an Activity Referral Form, or complete our self-referral form which can be accessed at either Selby or Tadcaster leisure centre.

Once you have received the form give our team a call on 01757 293286 and book in for an appointment at a centre close to you.  These centres include:

The initial consultation will last for approximately 45 minutes and instructor led activity sessions typically last between session 45 minutes and 1 hour. The programme is for 12 weeks, however we do offer on going support and reduced costs for a further year afterwards.

All activities are at a reduced cost of £2.50, as opposed to the regular Pay and Play price which is £5 per class. We do offer a number of free outdoor activities as part of our Health Walks and Active Outdoors programmes which are accessible as long as you're deemed suitable to take part (our instructors and volunteers can chat this through with you).
